Non-Credit Professional Programs Withdrawal Form

Students must petition the College for a withdrawal from a course if there are extenuating circumstances (i.e. deployment, extreme medical emergencies, natural disasters, etc.) by completing the withdrawal form and providing supporting documentation.

Non-attendance does not constitute withdrawal from a course.

Students may be terminated for failure to meet course requirements as determined by their instructor.

Please enter the Course Name, Course Prefix (Department), Course Number, Course Section, CRN, name of the Instructor, Start Date of the course, and whether the course is Online or On Campus.
