This form must be completed and submitted prior to the start of your first semester.
This form should be completed and submitted after the student has registered for classes.
Full-time teachers and administrators need only submit this form once at the time of admission.
Part-time and substitute teachers must submit this form prior to EACH semester for which they are taking courses.
