Tuition Reduction for Educators Form

 

Instructions for submitting this form:

  • This form must be completed and submitted prior to the start of your first semester.
  • This form should be completed and submitted after the student has registered for classes.
  • Full-time teachers and administrators need only submit this form once at the time of admission.
  • Part-time and substitute teachers must submit this form prior to EACH semester for which they are taking courses.

Part-time teachers must be under contract for current school year. Substitute teachers must have served as substitutes prior to enrollment and must document substituting at least 60 days during that semester with pay stubs or letter from administrator(s).
If you don't know your student ID, please leave blank.
Must involve teaching or direct academic student supervision.
(i.e. copy of contract or letter from principle/supervisor)
