College of Professional Studies Forms
The following policies and forms are for CPS students only.
Use this form to make changes to your degree program major or minor, including Declare/Change/Add/Drop.
Must be submitted during the add/drop period according to the current Academic Calendar.
Use this form to request complete withdrawal from the University. To withdraw from individual courses, use the Course Withdrawal form (WX) inside MyNova.
To be utilized during the advisement period to plan and outline registration procedures. Please click "Open in Acrobat" to enable e-signature capability.
Upon reaching 90 credit hours, meet with your advisor to draft a proposed timeline for graduation. Note: this plan is dependent on course offerings and student engagement.
Fill out this form to apply for CPS 3400.
Complete this form by March 1st to participate in May Commencement. This is not the Prospective Graduate Form (which is found in MyNova).
Students requesting more than 17 credit hours in a single semester (but less than or equal to 21). Students must have a GPA of at least 3.0 and must first speak with their advisor.
Students who have taken an incomplete (“N”) grade and need additional time to complete the work due to extenuating circumstances.
Students seeking to repeat a course where they initially received a passing grade of a D- or above, for courses taken at Villanova or transferred from another institution.
Submit in the semester in which you will be finishing the coursework for your certificate.
After speaking with your Academic Advisor, fill out this form to request permission to take courses at an external institution.
This form is for current degree or certificate program students only. If taking courses on an individual basis (not matriculated in an academic program), contact CPS admissions to get your application started. Deadlines apply to the semester in which you are completing your first credential/degree:
- For Fall Enrollment: July 15
- For Spring Enrollment: Nov. 15
- For Summer Enrollment: April 15

Scholarship Forms
All materials must be complete and submitted by March 1 (Summer and Fall semesters) or Oct 1 (Spring semester).
Adult degree-seeking students who have completed 24 credit hours, a GPA of 3.2 or greater, and have a financial need for assistance to complete their degree. Due by April 10th to cpsadvisor@villanova.edu.
Bisk Education, Inc. established the Bisk Education Endowed Scholarship for the College of Professional Studies to provide financial support to students who demonstrate financial need and academic potential. Candidates must also complete the FAFSA. See form for more information.
