Lost or stolen equipment form

If your university computing device is lost or stolen, please follow the steps below.  This procedure applies to all Villanova issued laptops, desktop, and mobile phones.

  1. Report the theft to Public Safety. Public Safety link to file a report.
  2. Complete form below to report the theft to UNIT. Please include your name, department, description of stolen euipment, and a budget account number. If you are unable to submit a service ticket, call the Tech Zone at 610-519-7777 to submit your request.

Departments are required to provide an account number at the time of the request; departments will be billed back for the cost of the replacement unit. Taking necessary steps to safeguard University property is each employee’s responsibility. Even if a laptop is stolen that was properly protected, for example: ( a locked office is broken into) affected departments are required to pay for any replacement equipment.
