Use this ONLINE FORM to submit a service request directly to the Facilities Management Office or Residence Life.

Tips for completing the form:

After you choose the building for which you are requesting service, choose ORL (Office for Residence Life).

You will receive a confirmation email upon your submission of this form. Keep this Work Request Number for follow-up.

Once your Work Request is submitted, your information is then verified by the Facilities Office or Residence Life*.

Call Comcast Cable Company at 1-855-638-2855 to report cable television problems.

Students must supply their own Coaxial Cable Cord for TV.
