Use this ONLINE FORM to submit a service request directly to the Facilities Management Office or Residence Life. After you choose the building for which you are requesting service, choose ORL (Office for Residence Life). You will receive a confirmation email upon your submission of this form. Keep this Work Request Number for follow-up. Once your Work Request is submitted, your information is then verified by the Facilities Office or Residence Life*. |
