Obtaining Medical Records
Medical records are kept by the Student Health Center for seven years following graduation or from the time the student left the University.
In order to obtain a copy of a medical record (e.g. Immunization History), please fax a request to the Student Health Center at (610) 519-4047. The request should include the following information:
- Your name, birth date, last 4 digits of your social security number
- Today’s date
- What specific information you are requesting
- The year you graduated or left the University
- Your daytime phone number and address
- Your signature
We will then process your request and send your records once we receive the processing fee. There is a $10 processing fee for this service. If there is no medical record on file, you will be notified as soon as possible.
